appeal u/sec 38, VAT + CST

Dear Sir

I have already paid your fee of Rs 500/- through internet, My Dvat registration has been cancelled due to refund default for the taxperiod 2013-14, for Ist Qtr and II Qtr. Please advise me how to file appeal under dvat 38 and ground of appeal.
