Here’s a Diagnostic Tool & Checklist designed to help you assess leadership effectiveness in your organization. You can use this tool to evaluate individual leaders or leadership as a whole, either through self-assessment, team feedback, or 360° reviews.

Section 1: Vision & Strategic Direction

Statement

Rating (1–5)

Notes

Leaders clearly communicate the organization’s vision and mission.

   

Strategic goals are aligned with the long-term vision.

   

Teams understand how their work contributes to the overall strategy.

   

Section 2: Communication & Transparency

📊 Scoring Guide

  • 1 = Strongly Disagree
  • 2 = Disagree
  • 3 = Neutral
  • 4 = Agree
  • 5 = Strongly Agree

Interpretation:

  • 4.0 – 5.0: Highly effective leadership
  • 3.0 – 3.9: Moderate performance; needs improvement
  • Below 3.0: At risk; consider intervention or development plans

How to Use:

  1. Self-assessment: Leaders evaluate themselves against this checklist.
  2. Team feedback: Anonymous surveys using the same statements.
  3. 360° review: Combine feedback from peers, subordinates, and supervisors.
