The Limited Liability Partnership Rules, 2009 provides the following forms for the purposes of LLP Act:

Note: Form 7 – Application for allotment of Designated Partner Identification Number and Form 10 – Intimation of changes in particulars by Designated Partners are omitted with effect from 09.07.2011.

Rule 36 provides that every form or application or document or declaration required to be filed or delivered under the Act and rules made there under shall be filed in computer readable electronic form, in PDF to the Registrar through the portal maintained by the Ministry of Corporate Affairs on its web site www.mca.gov.in or through any other web site approved by the Central Government and authenticated by a partner or designated partner of the LLP for such purpose by use of a valid digital signature.  Where the documents are required to be filed on non judicial stamp paper, the LLP shall submit such documents in the physical form, in addition to their submission in electronic form, unless the Central Government, by an order, does not require the submission in physical form.

Among the forms LLP Form No. 2 and 4 has addendum in addition to the form.   LLP Form 8 has an appendix.

Some forms are to be certified by the practicing professionals.

• Statement is to be given either by Advocate or Chartered Accountant in practice or Company Secretary in practice or Cost Accountant in practice – LLP form No. 2 and its addendum – Incorporation document and subscriber’s statement;
• Certificate is to be given by either Chartered Accountant in practice or Company Secretary in practice of Cost Accountant in practice for the following forms:

• LLP Form No. 4 and its addendum – Notice of appointment, cessation, change in name/address/designation of a designated partner and consent to become a partner/designated partner;
• LLP Form No. 5 – Notice for change of name;
• LLP Form No. 8 and its appendix – Statement of Account & Solvency;
• LLP Form No. 12 – Form for intimating other address for service of documents;
• LLP Form No. 15 – Notice for change of place of registered office;
• LLP Form No. 17 – Application and statement for the conversion of a firm into LLP;
• LLP Form No. 18 – Application and Statement for conversion of a private company/unlisted public company into LLP;
• LLP Form No. 31 – Application for compounding of an offence under the Act;
• LLP Form No. 32 – Form for filing addendum for rectification of defects or incompleteness

• Certificate only to be given by a Company Secretary in practice – LLP Form No. 11 – Annual Return of LLP.
